Madden said police had received reports of the shooting at the school shortly before 10:30 a.m., and officers responded within minutes.
Killed in the incident were 53-year-old Smith, 8-year-old student Jonathan Martinez, and Anderson in what's being considered a domestic murder-suicide.
After killing Smith on the scene, Anderson turned the gun on himself and took his own life.
The teacher's daughter also confirmed to investigators that Smith never took his threats of violence seriously and that there was never any sense of danger in her relationship with Anderson, police said.
"We just had to hide under our desks and then the SWATs came to the classroom and they told us to put our heads up". She declined to elaborate, the newspaper said.
The couple divorced in 1997.
Gun control activist Shannon Watts, who founded Moms Demand Action for Gun Sense in America following the Newtown shooting, said her group will continue to push for regulation on gun access rather than metal detectors as a solution.
He also struck two students who were standing behind Smith.

"He told her he would throw her out the window". Burguan said Smith moved out of the couple's residence in mid-March and had been staying with family.
